It depends if you still want the matter heard in court, if not then you need to start a three step process, if you are not experienced enough for that then just complete the form and then start learning on this forum how to address the court when you get there to stay out of contract. You can sign it with an ‘All rights reserved signature’ to keep yourself in a better position. I can give you more info about the courts later.
