UK health services like the NHS and Dental services might go private in the (near) future, but this could relate to other services..

Is it possible to give notice of a fee schedule to a General Practitioner for example, who might charge £10 for an appointment then charge extra fees for a physical exam or simple surgery ? (provided they don’t set up health insurance)

You can give a fee schedule if you are being asked to perform in some way that provides them with a benefit (eg being asked to come in to get the clot shot, but that offer can be declined and a fee schedule sent for any further attempts to solicit your custom), if you are asking them to provide you with a benefit then you are the one who has to pay.
