Does anyone know about the form here?
Date of event box, do I put the day of arrival or day of the registration?
-
Just ring them to check they are usually very helpful.

Hi Ant_made. Just checking to see if you have received source documents back yet? We submitted our applications with document you have shared around 5 weeks ago at service nsw office
-
Hi Yort,
I only printed it off yesterday. The lady yesterday said it could take a bit. Keep us posted on your arrival. I’m in S.A. now, I went into our BDM to see if I could do it through there. You would think with all the technology of today and falling under the main umbrella, we could access through their local departments. Only when it suits them.
Did you do registered mail to make sure it got there?
Ant

I was lucky and It was all done at service NSW desk because I’m in NSW at the moment. However I will call BDM this week to make sure they are sorting it out and that the application arrived ok. I’ll keep you posted. I have to get my daughters source document from QLD so I reached out to BDM directly, regarding that but have not heard back yet.
Yes I would send via registered mail. Also I would use this little beauty as I think it’s a really cool way to play the game:

Hope you are having a good weekend. We just got smashed with wind, lightning, thunder and heavy rain, and now some blue sky in Adelaide.

Any more progress? As I was adopted via court order I need to chase up few other documents to satisfy BDM. I had to hold position to get details of what they require and I was actually going to keep standing my ground however they are the gate keeper and the info they requested was actually info I’m interested to obtain. So I thanked her and said I’d be back in touch.
When they first responded they asked why I needed docs and in some cases source documents are not available. I respectfully hit her with heap of questions, probably over done it a bit because I think her intentions was to protect me in case I didn’t know I was adopted. However all her next reply said was ” get these documents and get back to us”. 1 of my questions to her was
- You claim that “not always available for some births”, Can you provide evidence that I am “some” and not Troy Peter ;(O’Callaghan)
haha I’m having a blast learning who I am and putting it into practice, so much fun

I sent mine Monday just gone. I will wait until next week to see where it’s at and i’m also waiting to see if money has been taken out of my account for the processing fee. My other option is to go back home to the hospital I was born and get the record from there. I said I was doing it as a heritage project for my boys.
It is a good feeling having the knowledge to hold your position. I took my driver’s licence out of my wallet, so I wont pull it out. A police officer pulled me over last week as I hit a pigeon during the day and my light went out. I explain it, he ask for my licence and I said I don’t have one on me. I only gave my first name and he didn’t push any further, he was polite and went on his way. I almost felt like getting out and having a chat with him….almost. I’m sure my plates were run with my licence and photo on his computer. I’m still waiting to hear back from my second letter for an Expiation Notice. It’s a month tomorrow and not sure if it null and void, as I asked them to show me the contact that says I am your property, produce the names of any victims that I caused harm, to stop the unlawful conversion of my name and to stop any further harassment.
I hope you get your Registration of Birth soon Mate. 6 weeks is a long time. You would think with an adoption, the paperwork would need to be transparent and up to scratch for future references to cover all parties involved.

Hey Troy, I just remember about this conversation. I got my original registration/source document yesterday. How did you go with yours? 7 and half weeks to get it. I was getting close to calling and then I saw the money get taken out of my account a week or so ago and new it was on its way.
Hope you had a good start to the new year.
-
Howdy mate it only took my wife 6 weeks to receive hers. Mine is taking some time as I had to source documents related to adoption, which the adoption agency let me know that there is an 18 month wait time to get documents. I contacted the hospital and sourced all info they had their. I should receive this info next week at which point I will contact BDM again and ask they send the Registration of Birth otherwise I will be left no option other than filing charges against the individual for theft of my private property.
-
When reading about others dealing with BDM, they always say go as high up as you can and deal with them. Put them on notice if they don’t help out. Most say that they help out, because they know what they are doing. Now we are talking about it, it will probably come soon. You know how that works, the universe and stuff.
I hope you are well and smiling, and all the best for the year ahead.
-
unfortunately the adoption information they require is info relating to a Supreme Court judgment from the adoption. haha I knew I was adopted but wasn’t aware any of this had taken place.
Anyways I have now gone back to BDM and noticed them asking what their source of authority is that they are using to withhold my private property? etc etc. See what happens

Wow! The things you discover when you dig for them. I can’t believe the waiting time. Morag shared this PDF with me. Another avenue to take. It might be an easier one and get the ball rolling, while you are trying to get them to respond. All the best Troy and keep us posted.

Hi, Could you please tell me can I find the form online to get my original birth registration doc from QLD?
As I am now in NSW can’t just go in to pick one up.
Thanks!
-
I looked, but the site is different (of course it is, not making anything easy) and I could not find it. Someone else might know or ring Monday, and they will guide you on the phone. I was born in NSW and I did mine from SA. She was very helpful. It would be good to have all states with links on the post. Im sure there will be others. If you get it, can you post it? Cheers
all the best

Below is the response I got from an enquiry online… also attached is the form. Hope it helps.
Thank you for your email. The application you are referring to is for a birth source document.
I can assist you with the process on ordering the requested source document.
1. Complete the attached application form to complete and post to the Registry Office.
2. Provide your 3 forms of certified identification documents as per the Registry’s proof of identity requirements (refer to page 2 of the attached application)
3. Supply a signed cover letter outlining the reason you are requesting the original source document.
Total cost is $119.50
Once you completed the above steps you can post all required documents to:
Registry of Births, Deaths and Marriages
PO Box 15188
CITY EAST QLD 4002
Once the requested documents have been received here at the Registry, the application will be reviewed.
