In response to your email, the Statement by Supplier form is not valid at MCEC as you are not employed as a contractor, and we do not accept ABNs from our employees.
We do require both Tax and Superannuation declaration forms completed. If you could please complete the attached forms and return to us ASAP, it would be greatly appreciated.
