If you have received a summons, in the Affidavit when writing your name, on the line for the First Respondent and also in the box/line for the Deponent, are you required to write it exactly as it is written on the Summons? My thoughts are Yes for the First Respondent (e.g. John Middlename DOE), No for the Deponent where you can write your name in Title Case e.g. John Middlename Doe.
