  Live birth record/ live birth registration statement/source document

  Hi all,

  I’ve just had my first off spring and i was just wondering if anyone knows what the Western Australian live birth record/ source document looks like? Is it the cot card? Part 1 of the registering a birth application form? Or is it a completely seperate document?
