Registration form for the registration of birth
Posted by whitelight on September 29, 2022 at 12:41 pmHi, I’m trying to find information on the site that deals with attaining my registration of birth for myself and my family. Mark mentioned it in one of the introductory videos but when I searched the site I find it, I also searched that chat but haven’t found anything.

I can’t post information on your request because I am not a member of your group and I am still waiting t be approved for membership. I have got a PDF that I think could help you if you ask me privately I can post it to you.
-
Hi, sorry I’m new to this platform so still working out how to use it. I checked my ‘Pending Requests’ and didn’t see any. I haven’t been able to figure out how to direct message yet. Any help would be appreciated. Thanks
-
Hi Morag,
Thank you so much for all that you do. I will look now to see if your approval has been activated.
-
Hi I think you are trying to get in touch with me, you will have to send me a private message because I can’t provide any documents to you on this forum as I haven’t obtained membership I am still awaiting confirmation for that.
-
Click on my name and send me a private message.

I will do, thanks again for taking the time to respond

Hi Whitelight,
Have you been able to do this yet? You would go to “Births, Deaths and Marriages” (or whatever it is called in your state/country) and ask for the source document. This is the one that was filled in by your mother when you were born.
-
So I’ve managed to make a few steps forward. For my daughters born in NSW I managed to track down the legit online request form for the NSW source document, so I’ll be dealing with that this weekend. For me, born in South Africa I have been emailing with the SA embassy in Canberra and they have pointed me towards the hospital I was born, so that’s my net port of call. The final one will be dealing with Scotland for my wife. I also found out that when I receive these source documents I can ask my mother to write a power of attorney handing over control of my estate, this will be my final phase. Thanks for checking in.
-
Good luck with it. I applied for my source document at a local court house where they had never seen the form I printed straight from the government BD&M website. That was 2 months ago and no sign of it yet
-
I received my source document last year, but forgot to post an update on this thread. It was certified by the Registrar General. I now need to apply for my husband’s and our children’s birth certificates. I see some talk (USA?) of people sending their BC back to get authenticated. Does anyone know of a similar process here? Good luck with the process.
-
Well done.
What was the form you printed straight off the BDM website?
-
It was an application for the source document.

That’s great.
I’m searching for this or something similar for Victoria. I might need to change my search words.
Thanks heaps.

I got lucky. Lady at SERVICE NSW had done an application 8 months ago so knew exactly what we wanted. However I don’t want to share info until we receive documents in case they are wrong and I give you false info in the process.
-
Yeah. Good call.
And funny how they make it extremely difficult to find these forms. You would almost think it was intentional.
Tongue in cheek.
Kind of confirms that they are concealing all this remedy
It’s worth the exercise sometimes. The ones who seeketh, shall findeth, and be the knowledgeable ones.
-
It’s all about knowing the exact terminology. now I hear we should get these documents authenticated at state and federal levels to help secure our status.
-
Sounds like 2nd witness. You can’t be the judge in your own matter as Mark says
Notarized should be ok.
Even affidavit??? No??

Interesting read.
Interesting WORDS.
Birth Rego/Cert
Victorian Law Reform Commission
Introduction
1.1 Birth registration is a significant life event which creates a relationship between the individual and the state. A child becomes a person before the law1 when their birth is registered.
1.2 Obtaining a birth certificate is a further step in creating an individual’s identity. This step can only occur once a birth is registered. A birth certificate is the ‘instrument universally recognised as the fundamental evidentiary document establishing personal identification’.2 Without a birth certificate a person is unable to invoke their full rights as a citizen.
1.3 These rights include: enrolling at school or to vote; obtaining a passport, Medicare card, driver licence or tax file number; and accessing various government benefits.
1.4 In short, birth registration establishes the legal identity of a child by placing them on the state’s records, and a birth certificate is the formal record of this status provided to the individual by the state.
1.5 Both birth registration and birth certificates are fundamental to our existence as citizens in a developed society. It is often taken as given that these events occur routinely in a developed country. The Commission is concerned that this may not be the case for all sectors of our community.

Wow! All there in black and white, now we comprehend what we’re reading
-
It might be possible that this is where it all begins, in getting BACK into the private.
They even tell us it’s important so we become citizens and then we can access the benefits and privileges that go with it.
Have a read of “Fruit from the poisonous tree by Melvin Stamper” (which is actually a legal term).

And “behold the pale horse”.
Be careful.
It’s a very deep rabbit hole.

I don’t know if this helps, but for interest’ sake, before I found the source document application, I applied for a BC using my name only and received this BC with name as Lyn, no last name. I blanked out my Mum and Dad’s family names, and other details but the number is the year born/number from my source document, not the standard BC which is a 9 digit number. Not sure if that’s significant, but they used details from source doc and no capital letters. It still has a registration number though 🤔

Thanks for providing the PDF. This is where I started, I emailed the BDM email address multiple times but didn’t get a response, digging around in some other chat room on telegram I managed to locate the NSW form. Once I confirm this work, I’ll be happy to share with this community. I just found out yesterday that the hospital I was born at in SA has since shut down… the investigation continues!

G’day GANG!!
I’m in Victoria.
Really struggling to get MY source doc and My 2 kids even – 12 & 14yo.
Just wondering if getting a stat dec or affidavit from my Mum, regarding me being born would be useful, and if so, who should witness it?….I would also do my own affidavit……AND
…………..if doing the same for my kids is sufficient. IE, my wife and I to do stat decs or affidavits for our Kids births.
Thanks guys.
