    Hi Marilyn,

    The Hospital form is called Registration of Live Birth, and the one at Registrar (Vital Statistics) is called Certificate of Live Birth (COLB) or Statement of Live Birth (SOLB), it can also be referred to as A Certified Copy of Registration of Birth, Write in Fluro yellow diagonally, LONG FORM, across the application form when filling this one out and at the top write (pen) in capital letters, FULL CONFIDENTIAL COPY. Your Birth Certificate should come from there as well and that is a receipt to the SOLB/COLB, and it will have the Registration number on it. If you contact the office they should be able to point you in the right direction.

  • Hey Jeremy, If they do not have that original Live Birth Certificate/registration, then there is no Surety and the BC that is being held against you, making you a Trustee, is fraudulent. There is a Breach of Trust. The Registrar has a duty to perform as those details were left in their care and they are the Custodian. Maybe wording to that effect might make it reappear.

    So that is like our State or Territory I am gathering? We go through them, not the Federal Registrar General. Is the District where you get your Birth Certificate? As it should be the from the same place. The BC is like a receipt to the SOLB/COLB. It is a Trust on top of a Trust.
