ATO Declaration Affidavit Estate Name
Hi all,
This maybe a simple question.
In the affidavit declaration provided as part of the paid download it used the name of ‘John Henry Smith’.
My account with the ATO is John Smith only.
So, when filling out the template, do I replace John Henry Smith with JUST ‘John Smith’?

Which paid download are you referring to? Are you saying the name on your birth certificate is John Smith?

Birth certificate is John Henry Smith, but my ATO account is John Smith.
On the affidavit should it be John Henry Smith, or John Smith or include both?

If you are talking about the non consent documents then use the birth certificate name.
