Hi, I am similar to you i.e. My ATO account was missing my middle name. Mark said I should enter my middle name on the affidavit.

Hi Laura,
In Q&A session I asked similar question to Mark. My account that I set up with the ATO is John Doe. My full name is John Henry Doe. So in the ATO affidavit I needed to write John Henry Doe as this is the name of the Cesti trust. I hope this helps?

Thanks for helping out.
I will say I am a little confused when it comes to the bills of exchange.
Module four does not mention the 5 cent stamp but module 7 does. Also the process is different. I purchased module 4, but apprehensive purchasing again as I am not sure of the benefit.
Can you please help with explain why the difference and does anyone have a copy of a completed BOE?

Sorry for late reply.
Birth certificate is John Henry Smith, but my ATO account is John Smith.
On the affidavit should it be John Henry Smith, or John Smith or include both?
